In registering for this program(s), you acknowledge that certain RISKS OF INJURY are inherent to participation in on-ice and off-ice hockey activities. You warrant that the registered participant(s) is physically fit to participate and understand that the choice to participate brings with it the assumption of those risks and results which are part of these activities. I understand that photos and videos taken by Hockey Extreme may be used for promotional purposes. I agree that Hockey Extreme Incorporated shall not be liable for any injury to the registered player and/or loss or damage to his/her personal property arising from, or in any way resulting from, his/her participation in these activities, unless such injury, loss or damage is caused by the sole negligence of HOCKEY EXTREME or its employees or agents while acting within the scope of their duties.

Hockey Extreme reserves the right to decline the applicant enrollment in the school and terminate participation in the school, without refund for conduct by the participant detrimental to the best interest of the school and other students.
Cancellations and Refunds: NO REFUNDS or make-ups for sessions missed due to illness, holidays, competitions, injuries or management disputes. If you show up or not, your hours at the school are considered used. Hours are non-transferrable from one student to another.
NSF Cheque: An administration fee of $30 will apply.
